Issues that have historically been problematic for the Medicaid fee-for-service (FFS)

- Reimbursement rates are considered too low.
- Administrative burden is high and payments are slow.
- Providers are reluctant to participate due to:
- Fear of gaining "too many" Medicaid/SCHIP patients.
- Perceived stigma of caring for low-income people.
- Concerns regarding adverse selection or mal-distribution of patients, particularly in
rural areas.
