In her discussion of quality measurement at the September
1998 SCHIP Workshop, presenter Lisa Simpson reviewed key issues in choosing
a quality measurement strategy, which are summarized below.
There are three basic dimensions to consider in choosing a
quality measurement strategy:
- Accountability. Who is responsible for quality?—
the health plans? the providers?
Provider
Structure:
· % weekend hrs,
· days wait for WCC
Process:
· % immunized
· treated w/respect
Outcome:
Health Plan
Structure:
· # pediatricians
· health plan stability
Process:
· %immunized
· asthma care
· lead screening
· % injury counseling
Outcome:
· preventable
hospitalizations
State/Community
Structure:
· % plans
accredited
· educational campaigns
Process:
· % immunized
· EPSDT rates
· HIV education
· care for CSHN
· lead screening
Outcome:
· school
absenteeism
· injury rate
· % LBW
- Time frame. Short- versus long-term. Will quality
measurements be used for year-to-year monitoring or to plan longer term
improvements?
- Method. Standardization versus innovation. Although
standardization facilitates comparison across States, differences between
States and their SCHIP populations may necessitate innovative quality
measures.
