Patient Safety Portfolio

Prevent, mitigate, and decrease the number of medical errors, patient safety risks and hazards, and quality gaps associated with health care and their harmful impact on patients.

This mission is accomplished by funding health services research in the following activities: Patient Safety Threats and Medical Errors, Patient Safety Organizations (PSOs), Patient Safety and Medical Liability Reform, and Healthcare-Associated Infections (HAIs). AHRQ-funded patient safety projects draw from disciplines such as clinical science, human and work systems factors, and epidemiology, and approaches to accelerate adoption and implementation of safe practices that have successfully improved care. Projects within the program seek to inform multiple stakeholders including health care organizations, providers, policymakers, researchers, patients and others. The Patient Safety Portfolio includes:

  • Patient Safety Organizations, which collect and analyze patient safety events that health care providers report and provide feedback to help clinicians and health care organizations improve health care quality.
  • Patient Safety Culture Assessment Tools, which enable health care providers to audit their current patient safety culture, track changes in patient safety over time, and evaluate the impact of specific patient safety interventions.
  • TeamSTEPPS® (Team Strategies and Tools to Enhance Performance and Patient Safety), an evidence-based teamwork system which is designed to improve communication and teamwork skills among health care professionals.
