Project Description: This research will develop
and pilot test a questionnaire designed to assess barriers to self-management
for persons with multiple chronic medical conditions. Based on results from
the pilot, Dr. Bayliss will refine the questionnaire into a clinically useful
tool. Finally, she will conduct a small, multi-method feasibility study in
which the clinical tool is used by providers in clinical settings.
Career Goals: Dr. Bayliss is a clinician
investigator at Kaiser Permanente, Colorado and Assistant Professor at the
University of Colorado Health Sciences Center. She is a board certified family
physician and has completed a primary care research fellowship and M.S.P.H.
degree. Her long term goal is to generate evidence-based recommendations for
the care of persons with multiple chronic medical conditions by identifying
issues relevant to the process of care for this population and by designing
practical tools and interventions to improve this process. Further, she would
like to encourage providers and policy makers to investigate alternatives to
exclusive use of disease-specific models.
Progress to Date: In addition to completion of
the survey of seniors with multi-morbidites, the grantee has conducted
interviews with 26 of the participants to explore their desires for care in
more details. Additional the survey in the renal disease population has been
used as an assessment tool in over 60 patients. Dr. Bayliss has submitted an
R21 application to AHRQ. She has articles in review and in press.
Future Plans: Work will continue with the
chronic kidney disease population. After all data has been collected, Dr.
Bayliss will assess the feasibility of using the tool in a clinical
environment.
