Project Description: The Extension for Community Healthcare Outcomes (ECHO) Model was developed by the University of New Mexico Health Sciences Center (UNM-HSC) as a platform for both service delivery and research. Using both state-of-the-art telehealth technology and case-based learning, ECHO can provide best practice care for complex health conditions to underserved priority populations while also training primary care providers. The implementation focuses on priority populations including ethnic/racial minorities, low-income patients, and rural areas with limited health care infrastructure. The proposed research aims to replicate the ECHO model and demonstrate its robustness for four distinct and complex health conditions by assessing its feasibility to deliver best practices, achieve positive clinical outcomes, and increase the knowledge of providers. The health conditions for the research projects are:
- Diabetes.
- Dementia.
- Adolescent Obesity.
- Opioid Addiction (Research on treatment with Buprenorphine).
