By now, your organization should have decided what the Patient Care Plan template will look like. It’s likely that you already have some format for discharge instructions, and these may or may not include all sections of the re-engineered care plan. You also probably have printed educational material for the disease or condition that you have targeted for this project. It is not necessary to re-create the educational material if it is well written, comprehensive, and in terms that the patient can understand.
Once the Patient Care Plan template is finalized, the DA must be able to access it and the required information that needs to be included in the plan. The DA must know how to print and save each unique Patient Care Plan.
