Outline for Project Presentation
As you are completing project work throughout the phases of your performance improvement (PI) methodology (e.g., Plan, Do, Check, Act or Define, Measure, Analyze, Improve, Control) it is helpful to create your final presentation as you go. The following outline will serve as a guideline setting up a presentation that may be delivered to staff, faculty, and senior leaders at project completion.
- Project charter.
- High-level process map.
- Primary physician referral base (pre-survey results).
- Patients' perception of the discharge process (pre-survey results).
- After hospital care plan structure.
- Performance improvement reporting structure.
- Team roles, definitions, and discharge planning rounds.
- Project metrics.
- Data collection plan.
- Cause and effect diagram.
- Lean waste and potential failure modes.
- Baseline data collection.
- Process for communicating discharge summary to primary care physician.
- Process for coordinating follow-up appointment and test results.
- Process for coordinating the post-discharge patient phone call.
- Plans for training and communicating the project to all staff and stakeholders.
- Process for brainstorming and prioritizing solutions specific to failure modes or root causes.
- Project deployment plan (three Ws — who will do what by when).
- Eleven components of Project RED.
- Collection of post-survey results (physicians, staff, and patients).
- Analysis of patterns and trends.
- Collection of pre- and post-project data.
- Monitoring plan for sustainability.
- Communication of results to the PI steering committee.
- Transfer of knowledge to hospital staff and the community.
