The Innovation

• The PFCC model at Magee-Womens Hospital of University of Pittsburgh Medical Center (UPMC) is a comprehensive, systems based approach that focuses on meeting the multiple, on-going needs of patients and families through the full cycle of care. The initiative, which was piloted within the Total Joint Replacement program, is organized around the patient's experience, including pre-surgical care, the actual surgery and post-surgical intervention and rehabilitation.

What is PFCC?

• Definition:
  
  PFCC, Patient and Family Centered Care, is a redesign of patient care so that the hospital resources and personnel are organized around the needs of the patients and families rather than around various specialized hospital departments. PFCC is a methodology to achieve the goal.

The bar graph measures "Total Knee Replacement" for total joint replacement (TJR) patients average length of stay (ALOS) at 2.8 days and for National ALOS at 3.9 days. "Total Hip Replacement" for TJR Patients ALOS is 2.7 days and for National ALOS, 5.0 days.

Exporting the PFCC Methodology

The PFCC methodology can be exported to any Care Experience, not just TJR. The following is a list of active working groups and areas in which PFCC has been successfully exported:
