Flowchart. Health plans and care systems and medical groups and clinicians provide data to data aggregation entity, which generates public reports and private feedback reports. Public reports are shared with patients and consumers for informed choice, purchasers for contracting accountability, and other public audiences for transparency. Medical groups and clinicians also provide data to patient registries for private feedback reports. Private feedback reports are used for quality improvement and patient care management.
