Appendix 9: Tools for Making Changes in Your Practice

Example Process Map for an Annual Health Assessment

Flow chart shows the Adult annual health assessment process for a paper questionnaire. The process begins with 'Patient checks in.' If the patient is under 18, has visited in less than 12 months or has had an acute visit, there is no health assessment (HA); if the patient does not fulfill these conditions, an HA is given. The MA reviews the HA, enters it into EHR template, flags 'positive' responses, and rooms the patient. The provider reviews the HA and decides whether further assessment is needed before conversing with the patient. If the patient agrees on priority, refer to care coordinator to arrange follow-up; if not, flag for review at next visit. The provider documents and codes the visit appropriately.
