Designing Consumer Reporting Systems for Patient Safety Events

A flowchart shows the structure of the consumer reporting system.

A consumer interacts with health care facilities and providers and experiences an event. The report is prepared and input into the system. Health care facilities and providers also add information about the event to reporting system records. The report flows to the systems processing hub. After processing, the report flows to systems outputs and feedback. Outputs and feedback flow to the consumer, health care facilities, other systems and organizations, and public reporting.
