Note 2: Mid-Atlantic and New England regions were combined to ensure hospital confidentiality. States and territories are categorized into regions by the American Hospital Association (AHA) as follows:

  • Mid-Atlantic/New England: CT, MA, ME, NH, NJ, NY, PA, RI, VT.
  • South Atlantic/Associated Territories: DC, DE, FL, GA, MD, NC, SC, VA, WV, Puerto Rico, Virgin Islands.
  • East North Central: IL, IN, MI, OH, WI.
  • East South Central: AL, KY, MS, TN.
  • West North Central: IA, KS, MN, MO, ND, NE, SD.
  • West South Central: AR, LA, OK, TX.
  • Mountain: AZ, CO, ID, MT, NM, NV, UT, WY.
  • Pacific/Associated Territories: AK, CA, HI, OR, WA, American Samoa, Guam, Marshall Islands, Northern Mariana Islands.
