Questions for Staff on Discharge Planning

To assist our hospital in assessing the impact of the Project RED (Re-Engineered Discharge) intervention, we are asking for your opinion on our current discharge planning activities. After Project RED has been implemented, we will ask for your opinion on the Project RED intervention's discharge planning activities.
