Generating the Discharge Care Plan

  • Manual - Use template for DA to enter all required data.
  • Provide template to your IT Department and request that they integrate with existing systems.
  • Purchas[sic] discharge planning software that is integrated with your existing systems.

Notes:

There are a variety of methods that can be used to generate the Patient Care Plan. We are hopeful that your process has been discussed and that you have a methodology to create your Patient Care Plan.
