The AHRQ template for the Patient Care Plan can be found on the AHRQ Web site. It is free and can be easily downloaded for your team's and patient's use. Some hospitals have had their information technology (IT) departments build interfaces that will feed certain patient information into this template. One area you may want to make a priority is a link to pharmacy to avoid retyping lengthy medication names for each patient. A pick list also simplifies the process and eliminates keystroke errors.
