This slide shows Boston Medical Center's (BMC) After Hospital Care Plan.  This document is provided as an example and can be used to cross walk the information that you are currently providing to your patients and the information that you may not be providing to your patients. The following slides will share the various components of BMC's After Hospital Care Plan. It is a great visual and can be used to assist your team in planning what you might want your Patient Care Plan to look like.
