This slide lists financial metrics that may be more difficult to capture. The first one may be available from your financial officer. For example, when a patient has been readmitted, what was the cost of that readmission? These dollars may be lost in the future as payers move to penalize hospitals for readmission rates that are different from national trends.
Project costs may be estimated as you consider the time project team members will invest in meeting, putting the care plan template in place, training, determining a PCP referral base, and establishing the process for post-discharge followup. A third cost, which may be the most difficult to calculate, is estimating the time staff spends in performing discharge planning activities now and later with the re-engineered program.
Once they're calculated, these costs can then be used to consider the return on investment of the new program.
