Project RED: Module 2: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 16

Provides a four-module training program to help hospitals implement Project RED.

Project RED Components

The 11 components enable DAs to:

  • Prepare patients for hospital discharge
  • Help patients safely transition from hospital to home
  • Promote patient self-health management
  • Support patients after discharge through follow-up phone call

Notes:

While this slide emphasizes the DA’s role as the primary coordinator and advocate for the patient, discharge planning requires multidisciplinary collaboration. The activities listed here are not possible without the involvement of the medical staff, case managers, pharmacists, and other therapists.
