Project RED: Module 2: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 20

Provides a four-module training program to help hospitals implement Project RED.

Sample Log to Track Key Dates and Times

Example of a log to keep track of new patients and when certain activities occurred.

Notes:

Here's an example of the Discharge Advocate's Data Collection Tool. The DA can use this log to keep track of new patients and when certain activities occurred. You may want to have staff charged with making the follow-up calls track the last item on this log, perhaps in a separate record.
