Project RED: Module 2: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 26

Provides a four-module training program to help hospitals implement Project RED.

Patient's Physician

  • Initiates patient plan of care based on critical pathway
  • Leads and participates in discharge planning rounds
  • Communicates potential date of discharge
  • Supports the performance improvement process

Notes:

The physician who provides care to the Project RED patient may be filled by an attending physician or a hospitalist who directs the care, treatment, and discharge activities of the focused patient population. Physicians are critical to the success or failure of the patient's hospitalization and his or her ability to continue care and treatment at home.

Establishing a collaborative relationship between the DA and the responsible physician will ensure clear communication pathways, mutual trust, and respect for the work that needs to be done for the patient.

If possible, one of the Project RED physicians should be a member of the project team, participate in meetings when able, and be kept informed and involved with the team's activities.
