• Reconcile admission medications with medications from home
• Collaborate with care team specific to discharge needs
• Reconcile medications upon discharge
• Assist with patient medication questions

Notes:

The pharmacist's role in Project RED not only includes the traditional responsibilities of verifying orders, reconciling orders, and establishing the medication administration record, but it also includes collaborative and teaching activities with the medical team and patients. The pharmacist may participate in the post-discharge phone call with the patient once specific questions pertaining to the medication schedule have been identified. Traditional methods of collecting data specific to medication reconciliation will be maintained within the hospital and pharmacy system. There should also be pharmacy representation on the project team.
