Project RED: Module 2: The Re-Designed Discharge Process: Patient Admission and Care and Treatment Education: Slide 30

Provides a four-module training program to help hospitals implement Project RED.

Notes:

You need to determine the role other therapists will play in determining post-discharge services and patient education. Physical therapy, occupational therapy, and speech therapy may all be part of the patient's health care team. If you have a disease management program for the targeted condition you've selected, then include them on the project team to ensure that patient education is well coordinated.

Also, consider the roles the admitting department and emergency department will play, particularly as you evaluate results of the Project RED intervention on emergency department visits.
