As we consider the initial steps of the re-engineered discharge process, let's take a look at the process map discussed in Module 1. By now, you should have completed your own process map. We've outlined the activities that Module 2 focuses on. As you can see, the various disciplines have identified their tasks; however, there are potential redundancies between the disciplines, there may be opportunities for collaboration and handoffs that are not documented, and there may be opportunity to eliminate lean waste. Once each member of the team documents his or her current state process, the team can begin to see what the swim lane process looks and feels like to the patient. Once all desired processes are understood, the future state can be mapped.
