• Confirm name of PCP with patient
• Add PCP name and contact number to Patient Care Plan

Notes:

One reason patients return to the emergency department or are readmitted to the hospital is that they have not followed up with a physician after discharge. This may occur because the patient does not have a PCP. This component of Project RED asks hospitals to confirm the name of and contact information for the patient's PCP and add it to the Patient Care Plan or try to find a PCP for the patient who does not have one.
