• Review how to conduct teach-back with patient and family
• Finalize process for making post-discharge calls

In Module 3, we discuss the specific activities of the re-engineered discharge program, focusing on what happens when the discharge order is written or is imminent. Objectives for this module address the structure and processes that support the patient after discharge, such as teaching and completing the follow-up call to the patient.
