Project RED: Module 4: Re-Engineering Patient Discharge: The Hospital

Provides a four-module training program to help hospitals implement Project RED.

Metrics for Target Population

Outcome metrics (readmission rate).

  • Financial metrics.
  • Process metrics — time stamps.
  • Process metrics — Patient Care Plan completion.
  • Pre and post data — frontline staff, physicians, and patient surveys.

Notes:

The final phase of the Measure phase is to determine what outcome metrics exist in the organization and which metrics need to be designed. PI projects are not research studies. Process and outcome metrics are collected as simply as possible at the beginning of the project so the data can drive the team's perception of reality. If the team enters the PI project without fully understanding data from the current state, there will be no baseline to assess improvements.

Module 2 offers several measures in the categories shown on this slide. You may want to consider using some or all of them to evaluate the impact of your re-engineered discharge process.
