  • Identified in Analyze step.
  • Rank in relative importance.
  • Brainstorm potential solutions for each root cause.
  • Prioritize root causes with solutions.

Notes:

Now you will move into the Check or Improve phase of the project. You designed a current state map, had the opportunity to design a Cause and Effect Diagram, and have a good idea of some root causes for discharge variation with subsequent readmission might be. As you implement Project RED, you will also implement solutions that your team designed to fix broken pieces of your process. For example, if your case manager or social worker does not cover weekends and there is no back-up plan, this piece of your system is broken from the patient's viewpoint. Your team must discuss all potential failures in the newly redesigned discharge process and ensure there are solutions and back-up plans for each one.
