"We have established a Project RED team to oversee this performance improvement project. The team set the vision for the re-engineered discharge process, set the expectation for improvement (e.g., 50 percent reduction in readmission rates), determined who the project champion is, and will receive and review project updates through a systematic process. It may become apparent that the senior leaders need to communicate project updates to the hospital board of directors or may need to create a standing project update within departmental management meetings. The Project Oversight Committee will make these decisions."
