Identifying Targeted Patients on Admission

  • How will you first identify that a newly admitted patient is in the targeted population for this project?
  • How will the Discharge Advocate (DA) be notified that a potential patient for Project RED has been admitted?
  • What secondary screening criteria for patient inclusion will the DA use to confirm the use of the Project RED intervention with the patient?
  • How will the DA track activities with new patients?


Note: Use this slide to inform the staff members how they are expected to participate in the identification of newly admitted patients who may qualify for the Project RED project. You may want to add a slide that specifically answers these questions for them or break this slide into multiple slides that show the questions and the answers. The question "How will DA track activities with new patients?" may not be relevant for staff, although it is an opportunity to indicate that the timeliness of this notification will be tracked.
