Project RED: Re-Engineered Discharge (Staff Training)

Provides a four-module training program to help hospitals implement Project RED.

Discharge Advocate

  • Project RED's 11 components let the DA:
    • Prepare patients for hospital discharge
    • Help patients safely transition from hospital to home
    • Promote patient self-health management
    • Support patients after discharge through follow-up phone call
