Notes:
"Clarifying how the DA works with the case manager is an important step in re-engineering the discharge process. Case managers are usually nurses or social workers who develop relationships with patients, arrange post-discharge services, and provide education on the patient's condition. The patient education that case managers provide need not stop, even though the DA provides daily education and the final teaching at discharge. The case manager and DA should discuss the educational content as well as post-discharge needs and services."
