Note: You can tailor this slide to describe the approach you will take for creating and storing the Patient Care Plan.

“Each patient receives a Patient Care Plan. Our DA knows where to access information to update to the care plan and will likely add to it every day. Once the discharge order is written, the DA completes the Patient Care Plan, saves it, and stores it. A copy of the Patient Care Plan is given to the patient and one will be sent to the patient's primary care practitioner.”

Note: If your hospital has decided to make the Patient Care Plan a part of the permanent patient record, say so at this time. Also mention that a copy of the Patient Care Plan will be sent to the quality department to measure the plan's completeness of the plan if you are using those metrics.
