Project RED: Re-Engineered Discharge (Staff Training)

Provides a four-module training program to help hospitals implement Project RED.

Post-Discharge Services

  • Confirms with case manager that all services have been arranged
  • Adds names and contact information of service providers to Patient Care Plan

Notes:

"Once the discharge order is written, the DA confirms with case management that post-discharge services are arranged. The DA adds service providers' names and contact information to the Patient Care Plan and points out this information during the final teaching session with the patient."
