Project RED: Re-Engineered Discharge (Staff Training)

Provides a four-module training program to help hospitals implement Project RED.

Primary Care Provider

  • Confirms name of PCP with patient
  • Adds name and contact number of PCP to Patient Care Plan

Notes:
