Project RED: Re-Engineered Discharge (Staff Training)
Provides a four-module training program to help hospitals implement Project RED.
Primary Care Physician Referral Base
- Leaders identify the PCP referral base
- Hospital assesses PCP satisfaction before project launch
- Physician champion communicates with PCPs about project
- PCPs advise how to handle off-shift and weekend patient needs
Notes:
"Leaders help identify their hospital's primary care physician referral base so that the hospital can help find a primary care physician for the patient who doesn't have one."
Note: Indicate what your plan is for patients in your project who do not have primary care physicians.
