• Poor discharge instructions
  • Patient doesn't understand how to use medications
  • Patient doesn't learn warning signs for when to report to their physician
• Poor information transfer
  • From hospital to primary care physician (PCP)
  • From hospital to nursing home staff
  • Lack of clarity on end-of-life care preferences
