Project RED: Re-Engineered Discharge (Staff Training)

Provides a four-module training program to help hospitals implement Project RED.

Diagnosis-Specific Reasons for Avoidable Readmissions

 

  • Post surgery
    • Surgeons not arranging for post-surgical primary care
    • Post-CABG patients, expecting to be pain free, seek readmission for angina
    • Inadequate patient teaching on self care after surgery (e.g., incision care)
  • Dialysis patients very vulnerable to drug therapy changes

Notes:

Here are some reasons for readmissions related to patients who have undergone surgery or dialysis.

Note: This may be a good time to ask the group to offer other diagnosis-related reasons for patient readmissions.
