Survey on Patient Safety Culture Database

Overview


In response to interest from hospitals using the AHRQ Hospital Survey on Patient Safety Culture, the Agency for Healthcare Research and Quality is establishing the Survey on Patient Safety Culture Database as a central repository for survey data.

Purpose of the Database

  • To gather data from the AHRQ Hospital Survey on Patient Safety Culture into a central repository to serve as an important resource for hospitals. Hospitals will be able to compare their patient safety culture survey results to those of other hospitals in support of patient safety culture improvement efforts.
  • To produce average scores and percentiles on the survey items and composites to help hospitals assess their own results to identify strengths and opportunities for patient safety culture improvement.

Benefits of Participation and Database Products

  • Aggregate hospital-level averages and percentiles will be available free to the public in a Chartbook to be posted on the AHRQ Web site. This document will not include any information identifying individual hospitals.
  • Additional database breakouts by bed size, teaching status, and other hospital characteristics will be available only to participating hospitals and health systems.

Participation Requirements

To be eligible for the 2006 Survey on Patient Safety Culture Database, a hospital or health system must have completed administration of the AHRQ Hospital Survey on Patient Safety Culture by May 31, 2006. The survey must have been administered in its entirety without significant modifications or deletions.

All types of hospitals are eligible, from acute care to rehabilitation and psychiatric hospitals, but the hospitals must be located in the United States or in a U.S. territory.

April 2006: Registration

Registration involves the following steps:

  • Step 1: Hospitals and health systems that have administered the AHRQ Hospital Survey on Patient Safety Culture indicate their desire to participate in the database by sending an E-mail with "Database" in the subject line to safetyculturesurvey@westat.com.
  • Step 2: Database staff send registration information.
  • Step 3: Hospitals:
    1. Complete registration information about their hospital.
    2. Send a copy of the survey(s) actually administered.
  • Step 4: Database staff review the registration information and survey to determine eligibility.
  • Step 5: Eligible hospitals/health systems will be given data specifications, asked to provide additional hospital information, sign a data use agreement (PDF file, 145 KB; Plugin Software Help) which assures confidentiality and explains how their data will be used, and asked to submit their survey data.

May 1-June 30, 2006: Data Submission

  • Eligible hospitals and health systems (or their survey vendors) submit their individual-level survey data and other required information about their institutions.
  • Hospitals and vendors will use a secure, Web-based system to submit their survey data online.
  • Data files must meet specifications.
  • The deadline for submission of all information and survey data files for the 2006 Survey on Patient Safety Culture Database is June 30, 2006.

July-October, 2006: Data Processing and 2006 Chartbook Produced

  • The data submitted will be processed, and averages and percentiles will be produced.
  • The first annual Chartbook based on the Survey on Patient Safety Culture items and composites will be free and in the public domain on the AHRQ Web site.
  • Additional database breakouts will be produced and made available to participating hospitals and health systems.

Year 2

A Year 2 Chartbook is planned for 2007 for data collections completed between June 1, 2006, and May 31, 2007. Specific information about Year 2 eligibility requirements and processes will be available later.
