Note: The American Hospital Association database was accessed in January 2003 to obtain electronic spreadsheets of the facility ZIP codes in the six-State region. Some States, such as North Dakota, do not have to supply facility location information to the AHA network database. Other States, such as Utah, did not have all ZIP codes for all facilities input into the network. For these cases, individual hospital ZIP-code information for each hospital was obtained from the AHA Guide to the Health Care Field 2002-2003 and used to develop our own spreadsheets.
