Risk Level	30-Day Mortality	Risk Class	Based on:
Low	Less 0.5 percent	I	Algorithm
Low	Greater than or equal to 0.5 and less than 1.0 percent	II	70 or fewer points
Low	Greater than or equal to 1.0 and less than 4.0 percent	III	71-90 points
Moderate	Greater than or equal to 4.0 and less than 10.0 percent	IV	91-130 points
High	Greater than or equal to 10.0 percent	V	Greater than 130 points
