This diagram describes the conceptual framework used, which is patient-centered rather than disease-specific. This framework also recognizes the role of health policy in influencing patient outcomes. All of the arrows in the framework are bidirectional, recognizing the multiple, complex interrelationships that influence health and function in older people: Policy; Financial Incentives and Health Care Organization; Clinical Practice; Community and Family, Individual Patient and Biology; and Function: Physical, Cognitive, Emotional, Role, Social.
