Text Description

This Exhibit shows the initial flow diagram for identifying surgical site infection (SSI) cases. The initial data inputs are at the top (electronic medical record [EMR] review or floor visit), each leading to verification step and an independent documentation step; a suspicious EMR review could also lead to a floor visit. Documentation leads to monthly aggregation of SSI cases that is used in reporting. Information used to identify suspected SSI cases include microbiology findings, other lab results, imaging, and hospital admissions data.
