^a Data were drawn from the Medical Evidence Report (CMS-2728) that is completed for all end stage renal disease (ESRD) patients beginning chronic outpatient dialysis. These data are stored in the Standard Information Management System (SIMS) database maintained by the 18 ESRD Networks. Data for 2009 and 2010 were drawn from the SIMS central repository on May 17, 2011.
