a Data were sampled from the CMS Reporting Hospital Quality Data for Annual Payment Update program, which includes data for adults 18 years and older who were hospitalized for one of four conditions: heart failure, acute myocardial infarction, pneumonia, and a subset of major surgical procedures. Data for 2009 include records for discharges that occurred from January to September. Data for 2010 include records for discharges that occurred from January to December 2010, excluding Maryland, Puerto Rico, and the U.S. Virgin Islands.

DSU - Data do not meet the criteria for statistical reliability, data quality, or confidentiality.
