a Severity of injuries is defined using the International Classification of Diseases programs for injury categorization (ICDPIC). The ICDPIC translates ICD-9 diagnosis codes into several standard injury categories or scores, including Injury Severity Score (ISS). An ISS of 16 or greater is classified as severe.

b Percentage of visits treated at level I or II trauma centers. Remaining percentages were treated at level III trauma centers or nontrauma units.
