a Consistent with the Agency for Healthcare Research and Quality Prevention Quality Indicators software, chronic obstructive pulmonary disease must be the principal diagnosis and transfers from other institutions are excluded.

b Rates are adjusted by age and gender using the total U.S. resident population for 2000 as the standard population; when reporting is by age, the adjustment is by gender only; when reporting is by gender, the adjustment is by age only. Rates prior to 2005 are not reported because of International Classification of Diseases, 9th Revision (ICD-9-CM) coding changes.
