|"I will continue to take the lessons learned from this experience to further improve on blood pressure care and control in my practice. The resources that were provided to my practice helped tremendously in improving and reshaping the hypertension control workflow."
– Dr. Charles King Chan
Helping primary care practices use evidence to improve blood pressure control is a key focus of AHRQ’s EvidenceNOW initiative, in addition to improving aspirin use, cholesterol management, and smoking cessation to prevent heart attacks and stroke among people at high risk.
|"I was able to improve on the workflow I have in place through participating in EvidenceNOW. The educational resources provided were extremely helpful when I counselled my patients, and also reinforced the team-based care model within my practice."
–Dr. Michael Richter
The selected Million Hearts® Hypertension Control Champions have worked with their patients to achieve blood pressure control rates at or above 70% through innovations in health information technology and electronic health records, patient communication, and team-based care approaches.
