Hypertension Protocol-Team Care with Medical Assistants
Resource: Hypertension Workflow (PDF, 113 KB, 1 page)
This resource provides an example of how a primary care practice can translate evidence on treatment for hypertension (high blood pressure) into a protocol for the primary care team, following Key Driver 1: Seek, select, and customize the best evidence for use by the practice. It describes the workflow to identify patients affected by changing evidence on hypertension treatment, part of Key Driver 5: Engage with patients and families in evidence-based care and quality. It also spells out the responsibilities of the care team, specifically authorizing medical assistants (MAs) to take on an expanded role, in alignment with Key Driver 4: Create and support high-functioning teams to deliver high-quality evidence-based care.
