Project Description: This research will develop and pilot test a questionnaire designed to assess barriers to self-management for persons with multiple chronic medical conditions. Based on results from the pilot, Dr. Bayliss will refine the questionnaire into a clinically useful tool. Finally, she will conduct a small, multi-method feasibility study in which the clinical tool is used by providers in clinical settings.
Career Goals: Dr. Bayliss is a clinician investigator at Kaiser Permanente, Colorado and Assistant Professor at the University of Colorado Health Sciences Center. She received her MD from University of Colorado and did her residency in Family Medicine at Mercy Medical Center in Denver. She went on to earn her MSPH from the University of Colorado while she was a NRSA Primary Care Research Fellow. Her long term goal is to generate evidence-based recommendations for the care of persons with multiple chronic medical conditions by identifying issues relevant to the process of care for this population and by designing practical tools and interventions to improve this process. Further, she would like to encourage providers and policymakers to investigate alternatives to exclusive use of disease-specific models.
Progress to Date: The grantee has published results of administering the survey to a geriatric population with multiple morbidities. She continues to collect data using the survey in a population of complex patients with renal disease and multiple morbidities.
Future Plans: Dr. Bayliss will continue to use the barriers assessment in the nephrology clinic and collect longitudinal data with plans to do a preliminary validation of the barriers assessment instrument as sensitive to change over time.
