Short profile summarize each project. The profiles include an overview of the transformation efforts that were studied and a summary of the key impacts and challenges, as well as the main lessons learned and implications for others considering primary care transformation.

Findings From the AHRQ Transforming Primary Care Grant Initiative

In 2010, AHRQ awarded 14 Transforming Primary Care Practice (TPC) grants to conduct retrospective evaluations of successfully implemented or ongoing patient-centered medical home (PCMH) transformation efforts in primary care settings. This report reviews the final TPC grant reports to collect information about the outcomes of the transformation efforts and lessons learned.
